Public DSCHC guidance says informal-plan enrolment can be done at the DSCHC head office at 10 DBS Road Asaba, at liaison offices in the 25 LGAs, through DSCHC field registration agents, or via the Android app.
Public DSCHC materials list the informal-plan premium as N7,000 per enrollee per year.

The public notice asks for CV, credentials, change-of-name evidence where applicable, birth certificate or age declaration, LGA-of-origin certificate, current practising licence where applicable, referee reports and two passport photographs.
